On behalf of the participants named on this application, for myself and all others, agrees to release indemnify all person and entities including Chapman University, City of Orange, their agents employees, officers or other operatives, from any and all claims for liability of any type of nature including personal injury whether directly or indirectly caused in the whole or in part by the releasers just named, arising out of or in connection with activities conducted by the Chapman University Alumni game, and further assumes any and all risk of injury to the participants occasioned thereby.
By submitting this application to Chapman University Alumni, I affirm that I have read and agree to the Alumni policy and hereby accept the terms of enrollment described above. Furthermore, I agree to pay Alumni cost/donation and authorize Chapman Alumni to charge my credit card. Credit card charges will show on your statements as "Panther Soccer Camp." |
